Devils Lake, ND		The early 6.0's especially. I guess the piston skirts were a little short. The piston would actually "rattle" when cold. GM bumped the engine warranty up to 180,000 mile on motors with this problem because people were complaining.

My vote goes to manifold or manifold gasket.
