| I had a client with electric radiant heat that periodically would blow holes in the heating elements. These were enclosed in steel tubes insulated inside with aluminum oxide. Trouble was the insulation was contaminated with bits of minerals (likely from steel plant slag) that became conductive when running hot. It took a lot of lab analysis to identify the problem.
The client didn't like those elements blowing out bits of red hot metal on the upholstered furniture they build.
