Do you need the riser/tower for it?
Not sure if they are available?

First of all, the valve and stack plate are the same, 4040 thru 4840. The linkage added to the console is pretty much the same also with exception of the external operating arm and connecting rod to the valve. Worthington and All States show complete kits in there catalogs, do not know how available they are. Get in a parts book and know EXACTLY what you need and visit a local salvage yard maybe. The valve assembly is the same as the lower two, but has a different operating shaft for the 3rd valve position. Hope this helps.

The stack plate is what is difficult to find.
#4 in the picture, is used as a single remote support.
You will need it in the stack version which supports the #2 and #3 remotes. 
We needed one a few years ago,
Local repair shop found a machine shop who could fabricate one out of an aluminum block.

What is the double stack plate actually called?
Thinking about getting one.
I could use one to add a #4 remote on a 30 series tractor.

That will NOT work to install a 4th selective control valve. Only bolts to the right hand side of rockshaft valve housing.

Maybe this is why the repair shop had one fabricated.
We do have a 4th remote on one tractor.
Have to exit the cab and operate it from ground level.
The 4th only gets operated sparingly.
Hose clamped a cheap screwdriver to the control lever. 

The hardware to do this is in the 60 series Row Crop parts catalog. ''Quad Selective Control Valve Kit". 4th valve was always a dealer installed option on those tractors. It will show the correct plate used on the left hand side. No need for shade tree engineering. Most easily found on salvage 60 and 70 series articulated tractors.

There is a 3rd remote support on the 30 series.
Probably is usable for a 4th remote. 
There is a 4th remote listed for the 40 series.
There is a 4th and 5th remote listed for the 60 series.
Looks like, the control cable for the 5th does not go into the console. 
Looks like, both the 60 series support and the 40 series support, are listed on the 50 series. 
Apparently, one is for working around the rock shaft, according to the information.
I think that possibility, the 2 listed for the 50 series.
Covers all of the ones used from the 30 series through the 60 series.
