Knot on the top string only on the second knot usually means the lower string is hanging behind the twine finger when it retracts.
The most common cause by far is the twine finger bushing clogged with debris (or the bearing hanging if it has the ball bearing twine fingers) causing the twine finger to not slap all the way back. Pull the twine finger and make sure the bearing/bushing is COMPLETELY free.
If the lower slackers are sticky at all they will not pull the slack out of the twine when the needle retracts, this will leave enough slack that the twine will stay behind the twine finger. Pull enough slack that they can swing through the full arc and check for any drag.
Check all the twine finger adjustment, make sure it does not go beyond 90 degrees
Check for damage on the back of the twine finger that would cause the twine to catch.
