Supreme and Premium require the RFV and other test results so dairies and the like can balance a ration. All the visual characteristics of Supreme hay are secondary to test results and most will have a test run on their own as well.
"Horse" alfalfa is generally supreme, but doesn't have to be. It has to be green, green, green.....leafy, smell and look good, bloom is acceptable to most but not seed. It can actually test in the Good range and still bring a premium price based on appearance. This is why many hay producers do not like dealing with 'short-on-cash' horse folks.
