I know a guy that was in the hospital business when this all started 50 years ago.  At that time Medicare paid about 90-97% of the actual cost of providing a service, you had some private no-pays, and so all the remaining cost recovery or profit for future upgrades or investors for a medical system had to come from private payers.

I too wonder how this compares today.
