Need to take shims out to tighten clutch, equal amount of shims removed from each set, lever should "snap in" but not overly tight, maybe 5-10 lbs push on lever,when it is engaged. How loose or worn out is the brass throw out bearing? Throw out bearing requires a little grease (1 shot) if is being used very much. How about the washer on the shaft and yokes, are they still in good condition?
Are the clutch plates greasy or dry, need to be dry.
hopefully some body else might have more ideas. You may have to remove and replace the cover 2-3 times to get the adjustment right.

Edit: don't hold lever down with your hand or foot, last I knew that throw out bearing was not cheap, and it won't last long with constant pressure on it.
