Phos is screaming for help in your results. In my opinion nothing else mentioned in this tread matters until that is taken care of.
Remember that beans remove a great deal of P, approx. 0.8 lbs per bushel of yield. In my experience a lot of folks underestimate how much P beans consume and remove.
