I've encountered this problem a few times and every time the piercing mechanism that pierces the top of the k-cup has been clogged with coffee dried up grounds. My fix includes removing the top cover (~4 screws), unhooking the rubber outlet hose and blowing compressed air through the piercing mechanism.
In the pic- the hose comes out of the Keurig and goes to the head where the piercing mechanism is. Take off the hose where it goes into the head (R.H. side of pic) and blow air into the head.
Note: Remove hose @ the base and blow directly into the housing. If you take a short-cut and connect to one of the hoses instead, it's possible the hose will blow if you use a little too much pressure. BTDT- The clear tube on the right is not OEM- it replaced the l-shaped opaque tube on the machine after i got a little aggressive with the air hose :^)
