I had what looked to be that except it was under and the same size as my watch.   Finally went to a dermatologist.  He said that I could apply all sorts of cream on it until the cows come home and it would do no good.    It's a fungus under the skin and in the hair follicles.   He prescribed some sort of an anti-fungal med that I took for six weeks as I recall.    Finally cleared up.
