The air handler blower should be on the inlet side of the furnace so it leaks into the firebox, and a big enough chimney will dramatically reduce the possibility of smoke and CO getting into the house.

Water is more efficient at moving heat, but an outdoor air furnace will work fine if you can place it at the end of the duct so it's a straight shot out, and a straight shot back, and close enough. A lot simpler to do everything with air, but limits the possibilities, no domestic hot water, floor heat, or multiple buildings.
