Insulin is the key.

Search for insulin resistance. And a person does not have to be fat to be insulin resistant.

People that do not store subceutaneous fat effectively will instead deposit fat in and around the organs. Liver, pancreas and muscles. This is the worst kind of fat and the dangerous to our health fat. TOFI. Thin on Outside, Fat on Inside. There is a small subset of the population that fit this description and they can be very unhealthy yet be thin, trim and atheletic with no signs of being very sick inside. They are some of the ones that drop dead of a heart attack and everyone wonders why, because they seemed so healthy. A doctor that specializes in diabetes said those type of diabetics are the most difficult cases. They can be really bad diabetic and never know it from outward physical symptoms. I never had that problem. Fat goes to my belly just fine.

Dr Benjamin Bikman, PhD researcher that specializes in insulin, a self proclaimed "insulin guy".
