One of our house cows was like that. When the power was out for 2 weeks a few years back(before we had a generator) we milked 2x a day to feed calves and dogs and cats. One, maybe two fingers and a thumb was NOT the ideal way to milk. Thankfully she had a bag like yours and didn't take too long to milk her. Glad my wife had smaller hands, she got milk maid duty. My doggone fingers would cramp up.

If ever there was a cow that you could say had too small of teats... that one would be it. Kind of makes it hard to tell if the calf has sucked for the first time.
