Compacted subbase is the most important.
Order concrete with a water reducer or plasticizer. Cost additional $4-8 per yard but takes a stiff concrete mix and let’s you pour it at a 6-7” slump. Concrete is speced to be poured at a 4” slump. Just adding water on site increases shrinkage and cracking. Keep concrete pad wet for 7 days after ( this is what eliminates cracking by letting it cure slower
