How often do you soil test and do you ever take tissue tests? You soil tests could be fine but due to imbalances in other nutrients, it could be limiting what is getting into the plant. A training i just attended, they talked how base saturation is not everything. There is a big relation between mg and k but it is a new concept they have just found out about.
