Digging out and tamping in quikrete would work good under the concrete, put some dirt on sides and get some grass growing.   Does the outlet always sit like that with standing water,  if so that is the real problem, and what caused the erosion.  Create a deeper trench or cleanout all muck in the trench so it can drain completely.
