They knew that a diaphragm prevented pregnancy in ancient Egypt, I think they had more than an idea that semen was involved. They didn't need to know about the little wiggly things to understand that male ejaculate going into the womb was the key point. Coitus interruptus has been used throughout human history. The demand for spermicidal pessaries in the ancient Mediterranean world drove the most effective herb to extinction.

A lot of knowledge has been lost and re-learned several times in human history. Scientific knowledge did not begin in the age of enlightenment.
