Yes it won't work with some cases and you may need an extension.
Regarding the charging issue you may need to make a change to the configuration of the Elf - by default the Bad Elf Direct Connect for the Lightning socket only allows 1A through to the connected iOS device. I do agree lighting cable would be better but they supplied the micro USB with mine so I was ok with it.
I have my iPad mounted in a piece of machinery and keep the charge cable constantly plugged in to it.
