Surface can be a problem. If it is a glass or highly reflective surface. Or one of those plastic mouse pads. Mouse pads haven't been "needed" since roller ball mice unless you are on a glass desk.
But to me it sounds like it is probably time for a new mouse if you can unplug and plug back in once in a while and it works again...
