Subject: 3 Q pump wiring Question

Northern Indiana

I have a 2 hp davis EMU pump from a lift station I want to wire it up to take water from my pond.
I am not familiar with the wireing schematic. Wires marked 1,2,3,4,5,and 40.

Motor is a t132 208 volt.

Anyone know how to wire this?

Yes, I have 3 phase service.
