Many reasons, a few have been mentioned. It can be as simple as a worn pivot pin. (It rotates and the worn spot goes to the front side or the back.) Brake coming apart, pivot arm wear, or the cab moving. This is if the actual spot that it engages at is changing. ( the pedal goes down to the floor some times but not others) If it stay at the same spot. Works sometimes not others, then I would look for the brake tangs being broke (grabs occasionally) or a oil leak.
