Here it's somewhat ambiguous if you order a “plain” cheeseburger as to whether it’s topped or not. Most fast food places will ask if you order a cheeseburger plain, if you want toppings.  The local restaurants will usually deliver the toppings on the side.

a few years ago I had went to the drive through of one of our local fast food establishments to grab a bite.  Went asked what I wanted, I answered, “Aww heck just get me a couple of plain ol’ cheeseburgers”.

To which, to my somewhat dismay the 20 questions started about what I meant by “plain ol’ cheeseburger”? Finally, and after alittle bit of scolding by the older order taking gal, I got my cheeseburger with ketchup and pickles.

so I guess the context in which it was delivered matters quite abit as what the final outcome is.
