If you're sure its engaging the correct clutch packs at the correct time (and only those clutch packs) and there's no consistency to which clutch fails, then I'd say you're on the right track and I would look into either the ECU getting bad data from the crank/cam sensor, or a problem on the CCD bus between the two.
This is assuming that I understand the cal process on a 20 series properly, I always assumed they used crank speed to detect clutch pack engagement. Wouldn't be something odd like measuring rail pressure to detect clutch engagement, could it?
EDIT: I was being a knucklehead and think of a 10 series. There's no CCD on a 20, and any CAN communication faults would've yielded a .09 code so that option is out.
