Sounds to me like improper washing and cleaning of the lettuce or salad material and utensels used to mix the salad up. People just dumped it out of the package onto the plate and ate it.
You are supposed to always wash off any fresh vegetables consumed that are not cooked to hopefully wash off any bacteria.
