In the places for more commoners, it can be anything from supply your own needs to anything in between.
In Egypt there was a guy or girl attending every bathroom (always gender specific in a 95% Muslim population), kept everything clean and tidy, and expected a tip. In Indonesia you were on your own.
Best to find out local customs ahead of time and be prepared. It varies. A LOT.
