My father has had 3 surgeries for his stenosis. He thinks he got a bad back from lifting too many lettuce crates and potato sacks when he was young. His first surgery was when he was about 57 and in the lumbar area. The next was when he was 67 and in the thoracic area and the final one was about 10 years later and in the cervical area. All three surgeries involved scraping out the openings where the nerves go through as they were getting pinched and squeezed to death.
His surgeries were successful and he was able to do an impressive amount of manual labor after the requisite recovery period.
I know of a couple guys who chose treatment options other than surgery but have not talked to them at length about it.
