Large elevation changes will affect your Ground Sampling Distance (Resolution) so the measureable distance on the ground within each pixel will differ. This isn't a problem as long as you are not measuring area across the variation.

You will want to start high and work low to clear any obstacles and terrain. You are correct, the software sets the home elevation at take off then goes up to the desired altitude and performs the planned mission.
