I have a great grandson that was adopted out 5 years ago. His Mom and Dad send us photos a couple times a year and he is included in birthday  and Christmas gifts along with my other 18 grandkids. He is aware that we are here but doesn't know what the relation is. It will be up to his parents to inform him of the situation when they feel that he is ready. I feel no urgency about it at this time and he is being well cared for and eventually it will all come together.
