I use to use 5% lime when I had it mixed at the feed mill. What does your gut feeling say to try? Lime is cheap. It acts as a limiter also, so I don't really know where to start. Probably 7%% salt and 5% lime. In my experience they will eat about 1# of a limiter like that per day, but I don'the have extensive observations, and always trial and error and never accurate weights or cow days measured.

I also struggle how to include mineral, but I don't know if it is critical for her last 60 days.
