Infections begin when the umbilical cord is wet and open allowing bacteria to get in to the body. Some cords do not close quickly giving bacteria more of chance. Then commonly these infections attack joints. Quite possibly it was just waiting till he was stressed and immune system compromised till it kicked in.
He will need longer consistent treatment.

Definitely give the vet a call may suggest another antibiotic that gets into joints better than what you have on hand or up dosage or treatment length
