The monitor goes into a test mode to start with and the 3 items you mentioned that show "test failed" have a problem. Probably a wiring problem. Look at the 2 ends of the main plug in and be sure that no pins (male and female) are pushed out of place so they are not makeing contact. I also spray contact cleaner in mine every time I hook the connection up.
