After sleeping on it, I am going to suggest that you ask/tell your "authorized CAT shop", that you want them to call/phone their CAT dealership(that allows them to sell CAT parts and service, and put a sign out front of their store) and have that foreman or mechanic ask for help from that CAT Dealership's TRUCK ENGINE TECHNICAL COMMUNICATOR. {or Truck T.C. for shorten version}

It is a free service , to them or you. That Truck T.C. can give them step by step instructions to troubleshoot this correctly, IN FACT, service personnel should be "asking for help", anytime it takes over 4 hours to t/s something on a CAT engine that comes into their shops.

Once, they prove it is or isn't definitely combustion gases(sight glasses), then they may want to force injectors, into a maximum fuel condition, while at idle, using a flat blade screwdriver on each injector's fuel rack, one at a time. This creates a fuel knock(= lots of cylinder pressure, for that one particular cylinder). This helps isolate and verifies a possible problem, WHILE a mechanic monitors the fuel return sight glass. (BUT, normally, this procedure is usually done AFTER we reamed all 6 coppers and reinstalled original injectors, BUT there happens to still a problem, when going on a test drive or chassis dyno)= It may be one on those rare conditions, of the injector tip, allows combustion gases back thru the injector, thus it actually aerates the fuel system in the head.

AGAIN, Get that CAT dealership's Truck Engine T.C. involved in this truck!
