Seriously, as honey cools and ages it crystalizes. I'm betting you are not emptying the squeeze bottle completely, leaving some crystals in the bottle. Once formed, these crystals aid in the formation of more crystals. Like you said a little heat and it all goes back to liquid. If you heat it to pasturize it (like commercial operations) it will take years to recrystalize. I like crystalized honey (you can get more on your bun).
