Maybe I'm missing something but this seems easier to me.
I'd make some new wheel nuts of whatever thread you are using now. But make the new nuts about 6" tall. Be pretty easy to make them on the lathe out of hex stock.
For the extension: I don't know if all thread is available in this size for a reasonable price. Could use bar stock and just turn the last few inches to whatever thread you need. Another easy lathe operation. Thread extension bars into your new tall wheel nuts. Some flats for a wrench on the extension bars would be nice when removing. Tall nuts can likely stay on all the time, even when the duals come off.
Like I said, maybe I'm missing something? I think that would work, no?
I don't know what happens when you're start pulling on an "empty" thimble. My gut says it should be ok but to try to avoid it. Why not thread a bolt into your "empty" thimble and tack weld that bolt into the thimble? Now you have a full thimble, and something to put a wrench on to prevent spinning.
